Changes to the Queensland Guardianship laws and forms

The Department of Justice and Attorney General (DJAG) has confirmed that new versions of the Enduring Power of Attorney (EPOA) and Advance Health Directive (AHD) forms have been approved for use from 30 November 2020 to coincide with changes to the law that commence on 30 November 2020. You can access the new forms/explanatory guides here.

Please note there is no transition period:

  • On or before 29 November 2020 – use the current AHD/EPOA forms.
    • witnessing of these forms must be completed by 29 November 2020
    • these forms CANNOT be completed after this date.
  • From 30 November 2020 – use the new AHD/EPOA forms
    • the new forms are not approved for use before this date.

The current versions of the forms will remain online until 30 November 2020.

After this time, DJAG will redirect that webpage to the new forms.
