Connecting End of Life
A Palliative Care Queensland project aiming to improve the capacity of health, social and community palliative care services within the community.
What is CELC?
The Connecting End of Life Care (CELC) has been designed by Palliative Care Queensland to improve systems, develop practice and build community capacity in relation to serious illness, dying, death and grief.
So far, we have proudly set in motion:
- Compassionate Communities
- Palliative Care Sector & Community training
- Health Professional Networks
- Compassionate Community Conversations
- and many other initiatives
A project with a lasting impact
The first Connecting End of Life Care project was in Townsville (CELC-T) and was administered by Palliative Care Queensland and originally funded by Northern Queensland Primary Health Network (NQPHN).
Whilst the CELC-T project has now completed, PCQ continues to have a strong presence in Townsville and in surrounding regions and continues the work of this project.
