Supporting Better End-of-Life Care Together
Join clinicians, care providers, and health professionals for a practical and informative mini forum focused on improving end-of-life care, communication, and support across our region.
This session brings together experienced experts and specialist services to share practical insights, resources, and approaches that support compassionate, person-centred care.
Each session includes a 20-minute presentation followed by a 20-minute Q&A, giving attendees the opportunity t learn from experienced presenters and ask questions relevant to their own practice.
Refreshments will be provided throughout the afternoon.

Advance Care Planning | Symptom Management & Medications
This practical session explores key aspects of palliative care planning and symptom management, including approaches to advance care planning conversations, common symptom presentations, and medication considerations in end-of-life care.
Ideal for clinicians and care staff looking to strengthen confidence in day-to-day palliative care support.

Session three – Blue Care: 1:00pm–1:50pm
Barriers & Challenges in Palliative Care in Regional and Remote Areas
Providing quality palliative care in regional and remote communities comes with unique challenges. This session will explore barriers to access, workforce and service delivery pressures, coordination of care, and the realities faced by patients, families, and healthcare teams outside metropolitan areas.
All Health Professional are welcome: Nurses, General Practitioners, Allied Health Professionals, Aged Care Staff, Community Care Providers, Pharmacists, Support Workers,
Students & Health Professionals interested in palliative care.
Session four – Mackay Hospital and Health Service: 2:00pm–2:50pm
Recognising Deterioration and the Dying Phase
This session will focus on recognising deterioration and identifying the dying phase, with discussion around communication, clinical indicators, and supporting patients and families with compassionate, person-centred care.
